Documents You Will Need to Apply for Medicaid

In addition to filing out the Medicaid application paperwork,  you will need to gather for the Illinois Department of Healthcare and Family Services the following documents/paperwork in order to apply for Medicaid benefits.  You need to basically provide financial information about the Medicaid applicant for the last 3 years (36 months).  You may need to contact financial institutions (banks, credit unions, etc.) to get financial statements for such time  period or to get copies of checks of over $500. 

1.  Financial statements for the last three (3) years {36 months} of present accounts (and for financial accounts, investment accounts and certificates of deposit which were closed during such time period and documentation as to where the money went when accounts or certificates of deposit were closed).  The period of 36 months is changing to 60 months. 

          (a)  Checking accounts

(b)  Savings accounts

(c)  Money market accounts                

          (d)  Certificates of Deposit

(e)  Investment accounts

          (f)  Tax returns for the past 3 years

2.  Explanation of deposits into and withdrawals from financial accounts if over $500 (five hundred dollars), but no explanation needed for deposits of social security income.  You may need to provide copies of the checks written for $500 or more.  

3.  Copy of the applicant’s Social Security card and applicant’s Medicare card

4.  Copy of the applicant’s driver’s license (or State ID card) and applicant’s birth certificate

5.  Copy of the applicant’s Blue Cross/Blue Shield, AARP, or other health insurance cards

6.  Proof (check, benefit letter, etc.) of Social Security income

7.  Proof of pension income (if any), such as a letter from the company issuing the pension checks which state the amounts of such checks

8.  Proof of other income (if any), such as community spouse’s income if s/he is employed

9.  Proof of any long-term care insurance benefits that are available to pay for the nursing home resident’s care

10.  Health insurance policies (with proof of cost/premiums of the health insurance)

11.  Life insurance policies (with a statement of the cash value of life insurance policies) and information about any life insurance policies that were cashed in during the past 3 years

12.  Any pre-paid burial plans, funeral arrangements or cemetery lots (with an itemized statement of date of purchase and cash value).  Inform the IL Dept of Human Services if any burial plots are already used.

13.  Info about investments:

          Stocks

          Mutual funds

          Savings bonds

          Treasury bonds

          IRAs

          401k plans,

          403b plans

          Other investments

14.  Deeds and tax statements for property owned now or sold in the last 3 years (and the closing statements for any property sold in the past 3 years) – plus deeds regarding any time shares, mineral rights, timber rights, gas rights.  If you sold property, provide a copy of the closing statement and RESPA statement (also called a HUD-1).  {If you own a mobile home or sold a mobile home in the past 3 years, provide information about such mobile home.}

15.  Information about any gifts made in the last 3 years (or 5 years if gifts made from a trust or into a trust) – Gifts of money, stocks, bonds, autos, life insurance policies, real estate, etc.

Information about the gifts should include:

             (a)  When the gifts were made (month and year)

             (b)  Who received the gifts

   (c)  What was given (money, stocks, real estate, car, etc.) and the amount/value

16.  Information on any other assets or income that you might have (time share, mineral rights, gas rights, etc.)

17.  If you are a widow or widower, then provide your spouse’s death certificate

You will need to give copies of the following to the Illinois Dept of Healthcare and Family Services when applying for Medicaid benefits. You can use this as a checklist. 

“Community spouse” means the nursing home resident’s spouse (if such spouse is not also residing in a nursing home). 

 

_____          Copies of federal income tax return forms for the past 36 months

_____                    Copies of state income tax return forms for the past 36 months

_____                    Copies of checks over $500 (five hundred dollars)

_____                    Copies of monthly/quarterly statements from banks, credit unions, and other financial institutions for accounts/CDs/IRAs still open

_____                    Copies of monthly/quarterly statements from banks, credit unions, or financial institutions for accounts, IRAs, 401k plans, 403b plans, and certificates of deposit which were closed in the last 36 months

_____                    Copies of savings bonds

_____          A list of all the savings bonds, showing who are the owners, when the bonds were issued, the type of each bond, and the present value of each bond

_____                    Copies of treasury bonds

_____                    Copies of certificates of deposit

_____                    Copies of the pages from savings account passbooks

_____                   Copies of annuities

_____                    Copies of deeds to real estate

                    1.  where the nursing home resident or his/her spouse is an owner (partially or fully of real estate)

                    2.  where the nursing home resident or spouse has a “life estate”

_____                    Copies of deeds to any time shares

_____                    Copies of real estate tax bills

_____                    Copies of the vehicle title for any mobile homes you own

_____          Copies of closing statements for any real estate or mobile homes sold in past 36 months

_____                    Copies of any leases for property that you own and are renting out

_____                    Copies of paperwork verifying income (social security check, pensions, proof of community spouse’s salary if s/he is working)

_____                    Copies of vehicle titles to automobiles

_____                    Copies of paperwork for cemetery plots

_____                    Copies of paperwork for prepaid funeral or cremation plans

_____                    Copies of paperwork showing ownership of any burial plots, vaults, crypts or mausoleums

_____                    Copy of the nursing home resident’s Medicare card

_____                    Copy of the nursing home resident’s Social Security card

_____                    Copy of the community spouse’s Social Security card

_____                    Copies of life insurance policies

_____                    Copies of paperwork showing the cash value of any life insurance policies you or your spouse own

_____                    Copies of information about any life insurance policies cashed in during the past 36 months (when cashed in, how much money received, where did the money go)

_____                    Copies of information about any stocks or mutual funds sold in the past 36 months (when sold, how many stocks sold, how much money received, where did the money go)

_____                    Copy of the nursing home resident’s drivers license (if there is one) or state ID card (if there is one)

_____          Copies of any long-term care insurance policies providing coverage for the nursing home resident

_____                    Copies of health care insurance policies (such as Medicare supplement)

_____                    Copies showing the premiums of health insurance policies

_____                    Copy of the nursing home resident’s birth certificate

_____          Copy of marriage license (if the nursing home resident is married and the spouse is alive)

_____          Copy of spouse’s death certificate (if the nursing home resident’s spouse is deceased)

_____                    Copies of any loans papers (such as where the nursing home resident or  his/her spouse is owed money by a child or grandchild)

_____                    Copies of documents pertaining to any mineral rights, gas rights, timber rights, or royalties either owned by or due the nursing home resident or  his/her spouse

_____                    Information on any other income or assets available to or owned by the nursing home resident or his/her spouse
